Since everyone's health and safety is our priority, it is important to follow the instructions below and adopt safe and responsible behaviors for the good of all clinic patrons.

For Patients:

Instruction 1: If you are unsure that your symptoms are related to COVID, please complete the COVID-19 self-assessment by clicking the self-assessment link here before you arrive at the clinic.


Instruction 2: If you have symptoms related to COVID or if the self-assessment recommends that you isolate yourself, do not come to the clinic. Notify the reception that your appointment must be postponed to a later date.
Instruction 3: If you have no symptoms related to COVID or if the self-assessment tells you not to self-isolate, you can come to your appointment. Wear your mask before entering the clinic and for the duration of your presence at the clinic.
Instruction 4: When you arrive, go to the reception to complete the self-assessment of COVID symptoms (electronic version) which will be placed in your medical file at each of your visits.

These guidelines are part of best practices for preventing the spread of viruses and their variants and help identify potential community sources of infection.

For visitors and suppliers:

Instruction 1: If you have symptoms, do not come to the clinic, isolate yourself until you are symptom free.
Instruction 2: Please complete the COVID-19 self-assessment (click here for link to self-assessment) before you arrive at the clinic.


Instruction 3: Wear your mask before entering the clinic and for the duration of your presence at the clinic.
